At the whim of a crooked psychologist, two patients, who are electing gender reassignment, will get him to sign off on their surgery if they agree to a date that takes them all the way.

James and Donna happen to have the same psychologist assigned to them by the hospital. He holds the power to grant them permission to proceed with gender reassignment surgery. Using his authority illegally, he gives each an ultimatum: go on a date and go all the way. If they still feel the same after, he will sign off on their permission forms. James and Donna agree, seeing the end of the long road in their grasp. What they don’t consider, is that it only takes once to get pregnant. Donna now has to decide whether or not to keep, knowing it will push out her surgery if she does. James has to stand by, wondering if he will become a parent. To complicate things further, Donna hasn’t informed her parents of her intentions, while James hasn’t let his best friend from childhood know of his. Will James and Donna be able to live with their decisions? Will they lose those closest to them in the process?
